The one fact that decides this
Almost every Similan trip, whoever sells it, departs from Thap Lamu pier just south of Khao Lak. Guests picked up in Phuket and guests picked up in Khao Lak board the same boats, snorkel at the same stops and get the same hours at the islands. The only difference is how long you spend in a minivan. That single fact reframes the decision: you are not choosing between two trips, you are choosing how much of your day to spend on the road. A secondary pier at Ban Nam Khem, north of Khao Lak, is used by some operators.
What the Phuket day looks like
Pickup from a Phuket hotel is typically around six in the morning, sometimes earlier. Then a road transfer of roughly ninety minutes north to Thap Lamu, the crossing, the islands, and the whole sequence again in reverse, with hotel drop-offs usually landing between half past six and half past seven in the evening. Total elapsed time runs to ten hours and often more. It is entirely doable and hundreds of people do it daily through the season, but it is a long day bookended by two van legs, and small children rarely enjoy the second one.
What the Khao Lak day looks like
From Khao Lak the pickup is around seven and the transfer to the pier is short. You arrive at the islands at the same time as everyone else and get home in the late afternoon with an evening still in front of you. That is roughly eight to nine hours door to door instead of ten to thirteen. If the Similans are a significant reason for your trip, basing two or three nights in Khao Lak rather than day-tripping from Phuket buys back several hours and removes the pre-dawn start. Khao Lak is also where the dive centres and liveaboard operators are based.
When starting from Phuket makes sense
Phuket has by far the wider hotel range, the international airport, and everything else a trip might be built around; relocating for a single day trip is not always sensible. If you have one free day in a Phuket-based holiday, the Phuket pickup is a perfectly reasonable choice — accept the early alarm and the long return. It also makes sense when travelling with people who want only one island day and prefer to keep the rest of the holiday in one place. Seasickness is the main argument against it, since a rough crossing after ninety minutes in a van compounds badly.
Practical points for either departure
Confirm whether the national park fee is included in your price or collected at the pier, and carry baht in cash if it is not. Bring a passport or a clear photo of it, since the e-ticket is issued against passport details and rangers check codes on arrival. Reef-safe sunscreen is a legal requirement, not a suggestion, and a reusable water bottle avoids the single-use plastic ban. If you are prone to seasickness, take medication before pickup rather than at the pier, and choose a catamaran if one is offered on your date.
